Schedule An Appointment

To schedule an appointment please call 478-333-3612.  Our Office is open Monday to Thursday 8:30 am – 5:00 pm, and Friday from 8:30 am – 12:30 pm. Please be sure to have your insurance information with you when you call. You can also pre-register for an appointment through our Patient Portal here.
 
 

Welcome New Patients

To allow a smooth transition to our practice, Please plan to arrive 30 minutes before your scheduled appointment to allow time to complete paperwork. We encourage that you download and complete the new patient package here.
 
This will allow you a comprehensive visit with our providers. 
 
Please be sure to bring the following to your appointment: 
 
1. Insurance Card
2.  Drivers License or Photo Identification
3.  New Patient Packet (if downloaded as above)
4.  All Current Medication that you are taking, in their original bottles. 

Cancelling Appointments

If you must cancel a scheduled appointment, please notify us at least 24 hours in advance. Please understand our physicians are extremely busy and we may not be able to reschedule your appointment if you have cancelled or not showed up for your appointment more than two times in a row.

Telephone Calls

We try to return calls as quickly as possible. If you leave a voice message for one of our clinical staff, your call will be returned at the end of the morning or at the end of the day.
Calls received after 4 pm will be returned the following business day.

Laboratory Testing Services

Due to the possible complications related to the level of kidney function, our physicians may require you to do a “renal panal” during your consultation. Please also be prepared to offer a urine sample at your first visit. We offer onsite laboratory services at our facility. This allows convenience to our patients, and allows test results to be available within a short notice. 
We encourage you to register on our Patient Portal to receive lab and test results more quickly. If you are not registered on our Patient Portal, results will either be mailed to you or you will be called.
 
 

Prescription Refills

If you would like a refill on your prescription, please call our office and provide us details of, the name of your physician, name of pharmacy and the medication that you need a refill of. Please also check with your pharmacy as they may already have a refill request on file from your previous appointment. Please allow 24 hours for your request to be processed.
